Does someone have a picture of the EZ-Check form for LAX

Does someone have a picture of the EZ-Check form for LAX - interested in times and airlines that qualified. I realize it can change.

Is there EZ Check for LAX?   Thought it was only available at Fort Lauderdale.  A different program in Seattle.

I'm with Potstech - is EZ check available from San Pedro now? It's been only Fort Lauderdale for a while. And Port Valet in Seattle...

Gosh - my memory must be wrong. It has been a few years since I flew out of LAX - I was thinking there was but I usually take pictures of these forms and couldn't find one. There must not be.

No - I was thinking EZ-Check. I guess it has been a few years since I have ended a cruise in LA. Thanks everyone!
