No charge for any of the food in the showcase at Al bacio. Coffee is for a fee. It's all part of any beverage package and/or if you're Elite +.

Gelato cross the way isn't free except for a free scoop per cruise that might come come with your CC status.

I haven't seen any milkshakes unless you consider a frozen mudslide at the bar.

Captains Club members on their second cruise or more get a free scoop. 

After that it's no longer free.

It does seem to be included if you're in a suite that has unlimited specialty dining.  At least, that has been my experience when staying in such a suite and ordering more than once scoop over the course of a cruise (and maybe even two at a time 😁).
