Generally, yes.  As long as you're a US citizen, on a closed loop cruise starting and ending in a US port

 

That said, there are cruise lines that require a passport, regardless of the legal requirements.

Yes as long as you have a photo ID if you are over 16.

Just to reiterate in a single statement, for most cruise lines as indicated, the ID requirements for embarking on a closed loop cruise that departs from a US based port (which includes Puerto Rico) for US citizens over 16 is a birth certificate and a government issued photo ID -or- a Passport.  However, also as mentioned, there are cruise lines the require a Passport for these itineraries, so it is necessary to contact the cruise line in question to confirm their individual requirements.
