"filled out by me"? Ours were filled out by the person giving the vaccine.

Ours were left blank to be filled out by the recipient.  The only thing they did was put the label on the card. I will call the county as @Laminator recommended.   Who knows by time I sail next spring I might be getting another vaccine.

All this goes to show the differences between States, let alone Countries and Continents 🤔

However, it is still early days and some countries, mine included, are having difficulties rolling out the vaccinations to be too worried about certification to a level that hasn't even been agreed upon yet.

I have the same card.

I then emailed a copy to my medical provider.  And it was entered into my records.  Now all they (the doctor's office) knows is what I've provided.  In looking at my record in the doctor's files, it says "external provider".  No note of batch number or where.

1) If I were short of cash….thankfully I’m not…..I could sell this on the black market to someone (perhaps an anti-vaxxer?) who needs it for employment, travel, or whatever. I understand that this is actually being done.

2) Seriously? Does anyone expect that this could be used for proof of vaccination for my two upcoming booked flights/cruises? I think not!

 

I’m thinking that what I eventually will have to do for travel is to print out a copy of my actual vaccination history from my clinic’s web site. That will document my name, medical-record number, date administered, etc.

 

Yep, that our cards too, but our name and birthday were filled in by the vaccinating agency. but no official stamp or signature. Just the name of the agency filled in.

That is the standard card, to be completed and given to the client. It is supposed to have the person who received the vaccine’s name on it. 
 

There is standardized training that all who give the vaccines have to complete- For my Agency, there is additional training. I’m pretty sure the edict to complete the card before handing it to the client is in the CDC training. 
 

Frankly, I’d be bringing that to the attention of one of the Nursing Admins or clinic supervisors. Or contacting the Health Department jurisdiction where the Kaiser clinic is, and asking to speak with one of the Public Health Nurses about that. 😉

Well there was a lot of data entry and a preregistration form so maybe we need to do as @Laminator suggested and call our local agencies to get that digital record.  Thank you

Mine does have my name on it.  But not the name/title of the person who gave the shot.  Only the mass vaccination location.  Now, whether that information is actually all in my personal medical record, I'm not sure.

Ours is like this but still needs our second shot info in a few weeks.

I received my vaccinations fairly early on thru work. Our company provides specific specialist care to a large population of people with a chronic condition that is highly susceptible to adverse outcomes if they get COVID. I received the card as shown earlier. Recently when I went to my personal care provider with no affiliation to my company, she immediately had access to my vaccination records and it included the COVID vaccination information.
 

Also last year upon acceptance to college, my kid had to provide a vaccination record under an official seal which we easily requested online and had emailed directly to the college RN. 


I suspect some of us will have an easier time of this and others will attest it’s impossible to their last breath.

Many states, if not all, have some type of a vaccine registry.
 

Depending on your provider, they may have access to what is in your records. In my area of the world, ours is called CAIR2. If you get vaccinated at certain offices, you have no opt-out option. (The system is used to inventory and bill for vaccine administration.)

 

You can set some limits as to whether only your provider has access. This makes it easy for a school to confirm vaccinations. You should also be able to get a copy of your records from any participating office (depending on what their policies are).

Interesting.  When we got our shots we were given a yellow CDC Form which had our name, the vaccine information (including type and lot#) and date.  Our forms were stamped at the pharmacy and initialized by the pharmacist.

I already had a yellow International Certificate of Vaccination form with my yellow fever vaccinations for a cancelled South America cruise so had my COVID-19 vaccinations added to that in addition to the above mentioned CDC form.

 

The New Mexico Department of Health asked everyone to update the state's records if vaccinations were received through other than the state Department of Health registry. 

 

Hopefully, all who have been vaccinated will get a "certified" vaccination form that is recognized for international travel.
